If you are using a digital camera you have notes already taken for you through the Exif file attached to the image. This way you can note the settings once uploaded to the computer.

The new Adobe Photoshop has an Exif reader built into it but if you don't have that you can DL and use this program to view the file info:

If you are shooting "choked plastic" (film) then field notes are good but not always possible depending on the situation. You kinda get a feel for it and if its action you tend to let the camera do a lot of the work.

When shooting action shots like cars I tend to use shutter priority and slow the shutter down to 1/60th and let the camera pick aperature depending on conditions, then you pan with the vehicle as it passes.

Example:

Shutter priority: 1/60
Aperature: camera chosen

A tripod is a plus for this as your hand held methods are rarely ever stable enough for this type of exposure although I have done it several times and succeeded.

Hope this helps some. Let me know if you need more info or I can answer anything else
