Thanks for the help guys. Upon closer inspection of my CVs, I noticed that the outer end of the drivers side shaft binds up at certain angles. I think this caused it to pull part way out of the transmission. I found some metal shavings where the CV slides in, but didn't see any when I drained the transmission oil. I'm getting a new CV today and hope this cures the problem.
