Well, your NOX is through the roof so that points to the EGR valve. Rip off the EGR valve and clean it. Also, the EGR ports in the manifold are more than likely clogged, so scrub those clean as well.

New spark plugs (NGK ONLY), new distributor cap, rotor, and wires will help the HC and CO if those haven’t been replaced in a while. A new O2 sensor may be needed if that hasn't been replace in 60,000 miles or so. Check and adjust the timing.

Make sure the motor is FULLY warmed up before taking the test. Drive it on the freeway for 20 minutes or so. Keep the motor running and the RPM's between 1500-2000 RPM's if you have to wait for the test.

Either that, or it's been like that since sometime after your last smog test ;-) jk

But I agree with Dan. Do those maintenance items first. And yes, you should let the catalytic converter warm up. Did you know that while the catalytic converter is cold, it does absolutely nothing with your exhaust? The catalysts in the converter need some heat to work properly (hence why it's placed closer to the header). It's not placed super close because too much heat will damage it as well.
