Engine likes to loose responsiveness

Engine likes to loose responsiveness
Problem:
For the past two months or so, my car has had these occurrences where it would loose responsiveness and start bogging, slowing down, and possibly missing/misfiring (not sure how it feels/sounds) for a few moments.
When does it happen:
Basically it usually happens when it's hott outside or the engine is good and warm and I try to pull off from a dead stop. For instance; It usually happens like right after I have backed out of my drive way, shifted into drive, and proceed to go uphill on my street. The car will climb rpms fine throughout 1st gear, but then after that shift to 2nd the RPMs drop super low and it starts doing the problem described above for maybe 5-10 seconds..
Key Information:
The only way out of the problem is to either 1) Hold the pedal down where I have it and wait for it to stop acting stupid, or 2) let off the gas for a few moments and try again.
I can cut the possibility of it occurring if I hold the car in 1st gear and let the rpms climb higher than they normally would before I shift into 2nd... and I usually wait till I get to the top of the hill before I do this shift.
Right now I am classifying this problem as being intermittent becaue I haven't gotten a pretty solid idea of when it wants to act like this. The problem more or less goes away after I get off my street... by then I guess the culprit has been defeated, but it has occoured in other places too.. but not to such as servere extent as when leaving home. This problem only seems to occur during the day.
Not too long ago I put in a new fuel filter and that didn't help.
Checked my PCV Valve, cleaned the vaccume line for it.
Cleaned my intake filter and checked the tightness of the throttle cable... even tighted it and the slave cable to the auto tranny up some... that didn't eliminate the problem, just made my tranny hold gears longer which resulted in harsher shifts so I loosened them back up.
Added a bit of Seafoam trans treatment to the ATF.
Changed spark plugs from Denso to NGK.
Used fuel injector cleaner. (seafoam into the gas tank)
I'm wondering if doing a seafoam might help.. i've done it twice in the past but i've read some things lately about people's cars getting trashed by seafoam now I'm scared lol.
Please help, I appreciate your time. If you have any questions about my situation please do not hesistate to ask!!
